The closest real evidence comes from four adjacent sources: a perimenopause trial (women in their mid-40s to mid-50s), a postmenopausal trial (ages 40-55) that found improved bone density at a 500mg dose, general-adult stress trials with no age breakdown, and one 2026 trial in adults 60-85 that included both men and women. None of it is a direct, dedicated answer to "does ashwagandha work for women over 50" β€” this guide shows exactly how close each piece of evidence actually gets, and where it falls short.

For "women over 50" specifically, that distinction matters more than usual, because β€” stated plainly β€” it isn't a studied category at all.

The Honest Starting Point: "Over 50" Isn't a Studied Category

As the quick answer above states, that search came up empty. What gets marketed under this heading is, in every case we could verify, one of a few other things wearing an age-bracket label: general-adult stress and cortisol trials with no age stratification at all, perimenopause trials centered on women in their mid-40s to mid-50s (a real population, but not the same as "over 50" as most readers picture it), a postmenopausal trial spanning ages 40 to 55, or a single 2026 trial in older adults that isn't restricted to women. NCCIH's own ashwagandha reference page doesn't have a menopause or age-50-plus section at all, which is a useful, if blunt, confirmation that this is a genuine evidence gap rather than an oversight on our part (NCCIH).

That gap is worth naming directly rather than papering over, because "over 50" functions in most competing content as a pure SEO wrapper around evidence collected somewhere else entirely. This guide does the opposite: it shows you exactly which evidence is genuinely adjacent to this life stage, how closely it actually matches, and where the real gaps are β€” the same evidence-tier approach used throughout our guide to ashwagandha's scientific evidence.

What the Adjacent Evidence Actually Shows

Four categories of real research sit near this question, each relating to it imperfectly in a different way.

Perimenopause trials are the closest thing to age-relevant women's evidence in the ashwagandha research base. A 2021 trial gave 100 women (91 completing) with climacteric symptoms during perimenopause 300mg of ashwagandha twice daily for 8 weeks and found a significant reduction in total menopause-symptom scores, alongside increased estradiol and decreased FSH and LH (Gopal et al., 2021). A newer 2026 trial tested the same dose specifically in 60 women aged 45 to 55 over 56 days and found a highly significant reduction in symptom scores (Vani et al., 2026). We could not independently verify Gopal's exact participant age range from the freely available abstract, but "perimenopause" is itself a meaningful boundary β€” it describes the transition leading up to a woman's final period, typically in the mid-to-late 40s, which is a different population from a postmenopausal woman in her late 50s, 60s, or beyond.

One trial is genuinely postmenopausal, which is a closer match than perimenopause, even though its age range still starts at 40. A 2025 trial randomized 127 postmenopausal women aged 40 to 55 (123 completing, confirmed postmenopausal by 12 months without a period) across six arms β€” placebo, two doses of ashwagandha alone (250mg and 500mg, a Sensoril-type extract), two doses of Shatavari alone, and a combined arm β€” over 24 weeks. In the ashwagandha-only arms, the higher 500mg dose produced a significant improvement in lumbar spine bone density T-score versus placebo, and bone turnover markers moved favorably in a dose-dependent pattern (Pingali et al., 2025) β€” covered in more depth below.

General-adult stress and cortisol trials are the category most often mislabeled. None of the stress or cortisol research covered in our main benefits guide was stratified by age at all β€” it's evidence about adults generally, not about older women specifically, however a headline frames it.

One trial is genuinely age-relevant but not women-specific. A 2026 randomized, placebo-controlled trial gave 50 healthy adults aged 60 to 85 β€” 32 men and 18 women β€” 300mg of ashwagandha twice daily or placebo for 8 weeks. Quality-of-life scores improved significantly with ashwagandha versus placebo at week 4 and further by week 8, daytime sleepiness scores improved, and physical-fitness test sub-scales all improved, with no adverse events reported in the treatment group (Honnutagi et al., 2026). It's real, recent, peer-reviewed evidence in an age range that actually matches "over 50" β€” but a majority of its participants were men, so it can't be presented as women's evidence either. It's the best adjacent evidence available on age, not a direct answer to this exact question.

Bone, Muscle, and Healthy Aging: What's Plausible vs. Proven

This is the one section in this guide built on trial data that's actually closer to proof than plausibility, so it's worth walking through the numbers rather than just naming the trial.

In the postmenopausal trial above, the ashwagandha-500mg group showed a significant improvement in lumbar spine bone density T-score compared to placebo over 24 weeks, and bone turnover markers β€” CTX-1 and RANKL, which signal bone breakdown, and osteoprotegerin, which helps protect against it β€” moved favorably, with the largest effect at the higher dose (Pingali et al., 2025). That's a real, dose-dependent, ashwagandha-alone result (not mixed in with the trial's separate Shatavari arms) in exactly the population β€” postmenopausal women β€” this guide is about. It's a single trial, not yet replicated, so we're not calling it settled; but it's genuinely stronger evidence than most claims made under this keyword. The elderly trial above adds a second, separate data point: its Senior Fitness Test sub-scales β€” measures of strength, mobility, and balance β€” all improved significantly with ashwagandha over 8 weeks (Honnutagi et al., 2026), though again, that trial wasn't women-specific.

One more source is worth mentioning only with a clear caveat attached. A 2024 preprint posted on medRxiv describes a trial of 50 adults with a frailty score of 7 or higher (a mix of men and women), given ashwagandha root extract twice daily for 8 weeks, reporting improvements across a frailty scale, a 6-minute walk test, sleep quality, cognitive testing, and quality of life. We want to be explicit about what that citation actually is: a preprint, not a peer-reviewed publication. We searched specifically for evidence that it has since cleared peer review and could not confirm that it has (Naik et al., 2024 β€” medRxiv preprint, unverified peer-review status). Until that changes, we're presenting it as an unverified preprint only β€” worth knowing about, not something to treat as settled evidence the way the Pingali and Honnutagi trials above can be.

Thyroid Considerations for This Age Group

Thyroid conditions become genuinely more common with age, and specifically in women β€” a nationally representative U.S. survey found the highest prevalence of thyroid disease in adults 60 and older (15.4%), with a higher overall rate in women than men (Zhang et al., 2023). We're stating that plainly rather than implying a study exists where one doesn't. A Note on Hormone-Sensitive Cancer History

This is worth stating plainly because this guide already reports the mechanism: the perimenopause trial above found ashwagandha significantly increased serum estradiol while decreasing FSH and LH (Gopal et al., 2021). Breast cancer incidence rises sharply after age 50, and a meaningful share of readers in this age bracket are living with a history of estrogen-receptor-positive breast cancer or are taking hormone-blocking therapy such as tamoxifen or an aromatase inhibitor. No trial has tested ashwagandha specifically in women with a hormone-sensitive cancer history. Is ashwagandha safe for women with a history of breast cancer or hormone-sensitive cancer? This hasn't been tested in a dedicated trial. Ashwagandha has been shown to raise estradiol in at least one perimenopause trial (Gopal et al., 2021), so women with a history of hormone-sensitive (estrogen-receptor-positive) breast cancer, or who are taking tamoxifen or an aromatase inhibitor, should discuss ashwagandha with their oncology team before use rather than starting it on their own (MSKCC).

Can ashwagandha be taken with thyroid medication like levothyroxine? This combination hasn't been tested in a dedicated trial.
