If you take a prescription medicine and drink herbal tea most days, you have probably run into a warning somewhere and had no way to judge how seriously to take it. Most of those warnings are written about supplements and concentrated extracts, and a surprising number of them are about green tea — which is not a herbal tea at all. This guide sorts the question by drug class, because that is how the risk actually works, and says plainly where the evidence is strong, where it is a handful of case reports, and where there is simply nothing to worry about.
Why a food-level herb can still matter
Two things decide whether a plant in hot water can reach a dose that changes what a medicine does: how much active compound ends up in the cup, and whether the medicine has a narrow margin between "working" and "too much".
A cup of infusion is a weak preparation compared with a capsule. But "weak" is not "zero", and a few situations concentrate the risk:
- Narrow-therapeutic-index drugs, where a small shift in blood level matters — warfarin is the classic example.
- Absorption interference, where the tea and the tablet meet in your gut and one blocks the other. This is a timing problem, not a reason to stop drinking tea.
- Herbs that are pharmacologically active in their own right, of which licorice root is the one that genuinely catches people out.
- Daily habit at volume. One cup occasionally and four strong cups every day for a year are different exposures.
Everything below is organised around those four mechanisms.
First, a distinction nobody makes: green tea is not herbal tea
This matters more than any single warning on this page. Green, black, white and oolong tea all come from Camellia sinensis. Herbal teas — properly, tisanes — are infusions of anything else. The herbal tea benefits guide covers that distinction in full.
Most of the well-documented "tea and medication" research is about Camellia sinensis, and the effects are not small. In a randomised crossover study in 10 healthy volunteers, taking the antihistamine fexofenadine with a green tea extract solution containing roughly 300 mg of EGCG cut the drug's plasma exposure (AUC) by 70% (P < 0.001), apparently by blocking the intestinal transporter that absorbs it (Misaka et al., 2022). A review of green tea and cardiovascular drugs documents interactions with simvastatin, rosuvastatin, nadolol, sildenafil, tacrolimus and warfarin — while noting honestly that the warfarin and tacrolimus data come from single case reports, and that where proper pharmacokinetic studies exist the effect was mild to moderate with wide variation between individuals (Werba et al., 2018). The U.S. National Center for Complementary and Integrative Health makes the same point for patients: high-dose green tea can reduce the effectiveness of the beta-blocker nadolol, and green tea extract can reduce blood levels of atorvastatin (NCCIH Clinical Digest).
None of that transfers to chamomile or peppermint. If you have been avoiding your evening infusion because of something you read about tea and statins, it was probably the wrong plant.
Blood thinners and bleeding risk
This is the one drug class where herbal teas have a real, repeatedly documented signal.
A systematic review of warfarin interactions searched five databases through the end of 2019 and found 149 articles covering 78 foods, herbs or dietary supplements. Of those, 45 (57.7%) were reported to potentiate warfarin, 23 (29.5%) to inhibit it, and 10 (12.8%) to have limited impact. Twenty were linked to bleeding events ranging from gum bleeding and purpura to fatal intracranial bleeding. The authors' concluding list of items to be aware of names chamomile tea, green tea and ginger alongside cranberry, Ginkgo biloba and St John's wort (Tan & Lee, 2021).
NCCIH states the chamomile side directly: "Interactions between chamomile and some drugs metabolized by the liver and warfarin (a blood thinner) have been reported, and there are theoretical reasons to suspect that chamomile might interact with other drugs as well, such as sedatives" (NCCIH).
Two honest qualifications. Much of this evidence is case reports rather than controlled trials, so the size of the risk from a single daily cup is not established. And the reason it still deserves your attention is that the consequence — a bleed — is severe, and warfarin is monitored precisely because small changes matter.
What to do: if you take warfarin or another anticoagulant, do not start a new daily herbal habit without mentioning it. Consistency matters as much as avoidance: a cup a day, every day, is easier for INR monitoring to account for than three cups one week and none the next. Blood-pressure medicines — and the licorice trap
Ordinary caffeine-free infusions have no established effect on antihypertensive drugs. Licorice root is the exception, and it is a serious one.
Glycyrrhizic acid in licorice inhibits the enzyme 11β-hydroxysteroid dehydrogenase type 2, which lets cortisol act on mineralocorticoid receptors — producing sodium retention, potassium loss and raised blood pressure. This is not theoretical. Published case reports describe:
- a 49-year-old admitted with hypertension, facial and limb oedema and fatigue after three weeks of drinking licorice herbal teas, with potassium at 3.1 mmol/L (Cureus, 2023);
- a 79-year-old man with potassium of 1.8 mmol/L and metabolic alkalosis after daily licorice tea for 18 months — and no symptoms at all until routine bloods were run (Medicine (Baltimore), 2020);
- a 61-year-old woman with profound hypokalaemia, metabolic alkalosis and QT prolongation from excessive liquorice tea (Endocrinol Diabetes Metab Case Rep, 2026);
- hypokalaemia from a licorice-containing herbal tea in a patient also taking an SGLT2 inhibitor (Cureus, 2025).
The practical problem is that licorice is a sweetener in blended teas. It hides in "sweet" caffeine-free blends, digestive blends and throat teas under names like licorice root, liquorice, or Glycyrrhiza glabra. If you are treated for high blood pressure, take a diuretic, or have low potassium for any reason, read the ingredients panel rather than the front of the box.

Sedatives, sleep and anxiety medicines
The concern here is additive, not chemical: herbs with a mild calming effect stacked on top of a drug that does the same thing more strongly.
NCCIH describes the chamomile–sedative concern as theoretical rather than documented (NCCIH), and that is the right weight to give it. The same reasoning applies to valerian and passionflower in bedtime blends, which is why our caffeine-free bedtime teas guide flags them.
What to do: if you take a benzodiazepine, a Z-drug, an antihistamine sleep aid or a sedating antidepressant, notice how a strong bedtime infusion affects you before treating it as routine — particularly before driving the next morning. This is a caution about grogginess, not a prohibition.
Hormonal contraception
NCCIH is specific and it is worth quoting rather than paraphrasing: "Chamomile might have some of the same effects as estrogen. Preliminary studies suggest that taking chamomile along with birth control pills might decrease the effects of birth control pills" (NCCIH). The same statement appears in NCCIH's clinical digest for practitioners (NCCIH Clinical Digest).
"Preliminary" is doing real work in that sentence — this is not established, and no dose is specified. But given what an unreliable contraceptive costs, a question to your prescriber is cheap. NCCIH also notes chamomile might worsen conditions sensitive to oestrogen exposure, which is worth raising if that applies to you.
Absorption problems: iron tablets, thyroid medicine and other timed doses
This category catches far more people than the dramatic ones, and it has the easiest fix.
Iron. Polyphenols bind non-haem iron into complexes you cannot absorb. In a controlled study measuring iron absorption from a bread meal by radio-iron incorporation, herbal infusions were potent inhibitors: peppermint tea reduced absorption by 84% and chamomile by 47%, against 79–94% for black tea (Hurrell et al., 1999). The NHS medicine page for ferrous fumarate says the same thing in plainer terms — some food and drink affect how the medicine works, "these include tea, eggs or milk" (NHS). Levothyroxine. NHS guidance is to take it on an empty stomach, 30 to 60 minutes before your first meal of the day, any drink containing caffeine such as tea or coffee, or any other medicine (NHS). That instruction names caffeinated drinks specifically; the safest reading for a herbal infusion is to apply the same gap rather than assume an exemption.
Anything else with a timing instruction on the label. Antibiotics that chelate minerals, bisphosphonates and some antacid-sensitive drugs all come with spacing rules for the same reason.
How to time tea around a tablet
A simple, defensible routine that solves nearly all of the absorption category:
- Take iron tablets away from tea. Leave an hour or two either side, and take iron with something containing vitamin C instead if your clinician agrees.
- Keep levothyroxine first, alone, and unaccompanied — then wait the 30 to 60 minutes before anything else, tea included.
- Read the label for a spacing instruction before assuming a drink is neutral.
- Keep your habit consistent if you are on a monitored drug. Predictable is better than perfect.
- Check blends, not just herbs. The interacting ingredient is usually a minor component — licorice, or a caffeine-containing plant like maté — hidden in a blend named after something else.
When a cup is fine, and when to call the pharmacist
For most healthy adults not taking a narrow-therapeutic-index drug, one to three cups a day of chamomile, peppermint, ginger or rooibos is not an interaction problem, and treating it as one adds anxiety without adding safety.
Ask a pharmacist — who can check your actual medicine list in a minute — if any of these apply:
- You take warfarin or another anticoagulant or antiplatelet drug.
- You take a blood-pressure medicine or a diuretic and your tea contains licorice.
- You rely on hormonal contraception and drink chamomile daily.
- You take iron, levothyroxine or any drug with a timing instruction.
- You have kidney or liver disease, which changes how much of anything you clear.
- You are pregnant or breastfeeding — the data on most herbs is thin rather than reassuring, and NCCIH says outright that little is known about chamomile in pregnancy (NCCIH).

Your next step: write down what you actually drink — the herb, how many cups, how strong — and take that list to your next pharmacy visit or appointment. A five-word answer ("two chamomile teabags most evenings") is all a pharmacist needs to check it against your prescriptions, and it turns an open worry into a settled one.

Frequently Asked Questions (FAQ)
Can you take ibuprofen with herbal tea? There is no established interaction between ordinary herbal infusions and ibuprofen. The caution worth knowing is different: if you are also on an anticoagulant, NSAIDs already raise bleeding risk, and the herbs named in the warfarin review add to that picture rather than sitting apart from it.
How much herbal tea is too much when I'm on medication? There is no verified threshold, and anyone who gives you a number is estimating. What is defensible: a consistent one to three cups of a common culinary herb is the pattern the reassuring evidence describes, and a sudden jump to large daily volumes of a single strong herb is the pattern that shows up in case reports.
Do herbal teas interact with antidepressants? St John's wort does, extensively and dangerously — it is a potent enzyme inducer with documented clinically significant interactions (NCCIH Clinical Digest). It is sold as a supplement and sometimes in "mood" teas. Ordinary chamomile or peppermint is a different situation, though the additive-drowsiness point above applies to sedating antidepressants.
