Quick answer: There is no such thing as a look that confirms cortisol caused a belly. People asking this question are usually describing something real β weight that has settled around the middle while the rest of the body changed less β but that description fits abdominal fat gain from almost any cause. We have not published before-and-after body photos or stock "cortisol belly" imagery, and it is not an oversight. A photograph cannot show causation. Two images of the same abdomen taken hours apart can look substantially different because of posture, lighting, camera angle, time of day, what the person ate and normal daily variation in bloating β none of which is fat gain or loss. Comparison photos in this genre are typically selected precisely because they are dramatic, which makes them the least representative examples available.
More to the point, publishing an image captioned "this is what cortisol belly looks like" would contradict the only honest conclusion this topic supports. If appearance cannot identify the cause, then no photograph can illustrate it, and a picture presented as though it can is misinformation with a caption.
What can be illustrated honestly is anatomy β where subcutaneous and visceral fat sit relative to the abdominal wall β and that is described in words below.
What people are actually describing
The common description has three parts:
- Weight concentrated centrally, with arms and legs changing less.
- A firmer abdomen than the person remembers, sometimes described as "tight" rather than soft.
- A change that arrived without an obvious dietary explanation, often during a stressful period.
All three are real observations. None of them is specific. Central fat gain with relatively spared limbs is the general pattern of abdominal adiposity, and it is influenced by sex, age, genetics, the menopause transition, insulin resistance, alcohol intake and overall energy balance. Attributing it to one hormone is a leap the appearance itself does not support.
Is cortisol belly hard or soft?
This is the most-asked version of the question, and it does have a partial answer β as long as you are clear about what it answers.
Abdominal fat sits in two main places. Subcutaneous fat lies between the skin and the abdominal muscles: you can pinch it between finger and thumb, and it feels soft. Visceral fat lies deeper, behind the abdominal wall, surrounding the organs. You cannot pinch it, because the muscle layer is in the way. An abdomen that is enlarged but firm, where there is little to grasp, is more consistent with a larger visceral component.
That is anatomy, and it is a genuinely useful thing to understand. But note what it is not: it does not identify a cause. Visceral fat accumulates for many reasons. And firmness is a crude signal in practice, because abdominal muscle tone, bloating, posture and how recently you ate all change how an abdomen feels on any given day.
There is also a hard limit on what the hands can do. In a meta-analysis of 39 high-intensity interval training studies, abdominal and visceral fat losses were only detected by CT or MRI β less precise assessment methods missed them entirely (HIIT and total, abdominal and visceral fat mass β meta-analysis, 2018). If a scan is needed to detect the change reliably, self-assessment by feel is not going to be sensitive either.
What does cortisol belly feel like?
Beyond firmness, people describe tightness, a sense of fullness or pressure, and clothes fitting differently at the waist while fitting normally elsewhere.
One distinction is worth separating out here, because it changes what you should do. Fat does not fluctuate within a day; distension does. If your abdomen is noticeably larger by evening and flatter by morning, or changes markedly after meals, you are describing bloating rather than fat gain β a different mechanism with different answers. Persistent discomfort, pain or distension that does not follow a daily pattern is a reason to see a clinician rather than to reach for a diagnosis from the internet.
Cortisol belly upper vs lower
Both patterns are described online, and no evidence supports mapping either one to a hormonal cause. Where fat is deposited is shaped by sex, genetics, age and hormonal status, and it varies substantially between individuals who share the same underlying situation.
If you find a page assigning "upper belly" to one hormone and "lower belly" to another, treat it as invention. There is no validated scheme of that kind.
What does cortisol belly look like in women?
The honest answer is that it looks the same as abdominal fat gain in women generally β and for women in midlife, the better-documented explanation is usually not stress.
DXA data from the SWAN cohort followed 380 women for a median of 11.8 years and found android (abdominal) fat rising 5.54% a year during the menopause transition, against 1.21% a year beforehand, with visceral fat rising 6.24% a year having not increased at all before (Greendale et al., 2021). That is a measured trajectory tied to a life stage.
The same study contains a finding that matters for anyone trying to assess this at home: waist girth grew at rates that were not statistically different across the three menopausal stages, even while the scans showed visceral fat climbing sharply. Cortisol belly on a skinny person
This is the clearest demonstration that appearance is not diagnostic, which is why it deserves its own section.
You can carry a meaningful amount of visceral fat while looking slim and having a normal BMI. The pattern has a name β normal-weight obesity β and in a 2026 analysis of 9,290 adults with normal BMI, it affected roughly 1.8% to 2.3% depending on definition, and was independently associated with elevated triglycerides, an unfavourable triglyceride-to-HDL ratio and impaired fasting glucose (Normal-weight obesity and hidden cardiometabolic risk, 2026). A review of obesity epidemiology makes the underlying point plainly: BMI "has low sensitivity and there is a large inter-individual variability in the percent body fat for any given BMI value" (The epidemiology of obesity, 2019).
So a thin person can have the fat distribution people are worried about. What that does not establish is a cortisol cause β it establishes that neither the scale nor the mirror is measuring what you think it is.
The one case where appearance genuinely carries information
There is a situation in which physical signs matter, and it is not subtle.
In Cushing's syndrome β sustained, pathological cortisol excess β the recognised features include weight gain with thin arms and legs, a round face, increased fat around the base of the neck, a fatty hump between the shoulders, easy bruising, wide purple stretch marks mainly on the abdomen, breasts, hips and under the arms, and weak muscles. The NIDDK also notes that "not everyone with Cushing's syndrome has these symptoms, which can make it hard to detect" (Cushing's Syndrome β NIDDK).
Two things follow. First, this is the pattern that would justify concern about cortisol as a cause β a cluster of distinctive features, not abdominal weight alone. Second, it is uncommon: NIDDK puts endogenous Cushing's syndrome at roughly 40 to 70 people per million. It is also diagnosed with laboratory testing, not by appearance. What to measure instead
Since appearance cannot answer the question, use something that can be tracked:
- Waist circumference. Commonly used raised-risk thresholds are above 102 cm (40 in) for men and above 88 cm (35 in) for women. Measure consistently β same time of day, same point on the abdomen, tape snug and level.

- How things fit over months, not days. Daily fluctuation is mostly distension and fluid.
